We are located in Cantera Business Park, off of the I-88 and Winfield Road exit. Please make note that entry to the office building is from Ferry Road. 4320 is the building to the right.

Both evaluation and treatment sessions are 50 minutes each. To make the most of your evaluation session, it is beneficial to complete all intake forms prior to your first appointment.

Please make note, late arrival will result in shortened sessions. Shortened sessions are charged the same fee rate as full-length evaluation and treatment sessions.

We accept Blue Cross Blue Shield PPO, Blue Choice PPO, and Medicare Part B (traditional Medicare).

Superbills can be provided upon request for self-pay/out-of-network clients. Click here to learn more about how superbills work.

Please contact us to learn self-pay rates. Payment for self-pay clients is due at the time of service and is accepted in the form of cash and credit card. Prior to beginning therapy, a “Good Faith Estimate” will be provided, indicating an estimate of charges. Please click here to learn about The No Surprises Act & Good Faith Estimates.

For self-pay clients who wish to submit directly for reimbursement with their insurance provider, a superbill can be provided upon request.

If you are a self-play client whose insurance I am not in-network with, you may submit a claim directly to your insurance using a superbill. Click here to learn more about superbills.

We encourage you to become familiar with your out-of-network coverage prior to beginning therapy and inquire about your insurance reimbursement rates.

Attendance: Punctual and regular attendance is essential for benefiting from therapy. A pattern of multiple cancellations or no-shows will result in review of continuation of services and may require prepayment for future appointments.

Cancellation/No-Show: Cancellation must be made 24 hours prior to your appointment to avoid a late cancellation fee of $30. If you do not appear for a scheduled appointment, a no-show fee of $50 will be charged.

Late Arrival: Late arrival will result in shortened sessions. To avoid this, please arrive at the appointed time to receive a full 50-minute session. Make note, shortened sessions are charged the same fee rate as full-length evaluation and treatment sessions.

Rescheduling: Please reschedule canceled appointments promptly. Rescheduling is subject to availability.
